D is just another coefficient for the standard form of the parabolic partial differential equation. This particualr coefficient is multiplied by the first derivative of the function of interest (u'):

d*u'-div(c*grad(u))+a*u=f

In the case of heat transfer it is the ability of the material to store thermal energy (density times heat capacity):

rho*C*T'-div(k*grad(T))=Q+h*(Text-T)
