Math Acceleration

I am interested in finding out about the processes that districts use in order to accelerate students in math. Answers to the following questions would be greatly appreciated:
 At what grade level(s) are students identified for acceleration?  How are students identified for acceleration (assessments, teacher recommendation, etc.)?  Are students only accelerated one year or do you accelerate students two years? Three years?
