Pooled T-Test, Two-Sample T-Test

What is the difference between a pooled T-test and a two-sample T-test?
I have read what literature I have available, and have gleaned nothing.

NCSS Jr has a two-sample T-test procedure available, but not
(apparently) a pooled T-test. If a pooled T-test is appropriate, can I
always substitute a two-sample T-test instead?
