This Lesson Plan Library organizes dozens of math lessons and
units for grades 3-9, all aligned to the Common Core Standards.
The Chicago Lesson Study Group invites teachers to rate and
comment on these resources; and to join its forum to learn
about, and practice, lesson study as a way to steadily improve
student learning.

UK's Association of Teachers of Mathematics--some out of print

O'Brien and colleagues. O'Brien recommends software as well,
some of which is available free of charge.
