How to generate a Transitional Probability Matrix

I have to generate a Transitional Probability Matrix (TPM).

As far my data concern, I have Velocity-time profile (v vs t) and acceleration-time profile
(a vs t) data of a Driving Cycle.

Then, I have to generate TPM in the form of a two dimensional matrix with velocity and acceleration at current time (t(k)) for convenience. In each cell of the TPM, the probability
matrix of the velocity and acceleration at the next time (t(k+1)) are included.

Each cell represents the conditional probability,
P{V(k+1)=V(2),a(k+1)=a(2)|v(k)=v(1),a(k)=a(1)}, at a given cycle velocity and acceleration.
