Simscape/Stateflow hybrid dynamics system

Hi there,

Im trying to modulate a hybrid dynamic system with Simscape and Stateflow.
My probelm is, that when the state changes also the active subsystem, relevant for the modulation, changes. To make a continuos transition, I need to use some values from the first subsystem as initial values for the second subsystem.

As the "joint initial condition"-block has no input-port, I assume the only way to handle this is by using variables which can be changed during the simulation. But I cant find anywhere how this can be done...
