I wonder if you are thinking of the publication "Windows of Opportunity: Mathematics for Students with Special Needs." It is described as a "professional resource for regular classroom and special ed teachers in grades K-12 who teach math to special needs students" and "an excellent text for college educators."
There is an ad in the February 1995 issue of the Mathematics Teacher, p. 175. It says the book is edited by Carol A. Thornton and Nancy S. Bley. $34 for individual members (for non-members, $42.50). 8" x 10", Hardback, 528 pp. NCTM stock #550A2. Call 1-800-235-7566 to order.
