Teach thinking about thinking

My area of interest is grade 4 to grade 7 students. I would like to
know if anybody could help me with first hand, practical classroom
experience in teaching students effectively to think about their
thinking when solving real-life mathematical problems {word problems}.
Perhaps someone introduced an effective program, running for months,
to teach students to plan and execute steps to solve problems -
without too much guidance from the teacher. Someone who successfully
managed to cultivate problem solving attitudes in students - 9 to 13
years of age.
