It is an updated version of Chapters 1 - 11 as they used to be available at this site; the table of contents with ps and pdf files of Chapter 11 (corrected version) are still available, and the author has prepared a description of the content with a few examples. A table of errata is available in tex, dvi, postscript, and html versions.
