Secondary Math Interventions for students far behind

I work for a reform support organization and I am currently looking for examples of both targeted and comprehensive intervention strategies / programs for middle / high school math students that are behind or far behind their grade level. The research team of NCTM pointed me in your direction and thought you might be able to help me with my search. Can someone help?
