Subject: Survey

I am a graduate student at Cambridge College and a
teacher in Massachusetts. I am currently studying the effects
teachers have on their students' (7-12 years of age/ second
through sixth grade) regarding self-esteem and academic
performance.

I am looking for teachers who would be willing to take
ten minutes of your time to fill out my survey. If you are
willing to complete the survey, I would like to assure you that
the results of the survey will in no capacity identify the
school or the teachers who participated.
This is an ANONYMOUS survey.
