Logistic Regression with Random Effects

Hello.
We are trying to locate a source of information on the use of random
effects in logistic regression, where the response is both polytomous
and ordinal.
In particular, any discussions that involve the modification of the SAS
macro GLIMMIX to the task would be very much appreciated.
Replies will be compiled and posted.
